Making an allegation (complaint)


To make an allegation, please provide the College with the following information:

Patients making an allegation are requested to provide the College with their name, mailing address, telephone number, and MCP number.

A person making an allegation on behalf of a patient is requested to provide the College with their full name, mailing address and telephone number and to describe his/her relationship with the patient (parent, spouse, child, other relative, lawyer, friend, physician). The person should also provide the College with the patient information requested above.

A person making an allegation, on behalf of a patient will also be requested to provide the College with consent from the patient or the patient's legal representative for release of patient medical information to the College.

People making allegations are also requested to inform the College of the full name and, if known, the address of the physician against whom they are lodging a complaint.

People making allegations  are also requested to provide details regarding their allegation including:

  • Dates and locations of visits to the physician;
  • Reasons for consulting the physician;
  • A description of why there is concern regarding the physician's care, behaviour, etc.;
  • Names of other persons who were present during the physician visit;
  • Names of other physicians who were consulted regarding the matter.

For reasons of confidentiality, the College requests that allegations be submitted by paper mail and not electronically. Also for reasons of confidentiality, the College will only conduct an investigation by paper mail.
